Health and Wellbeing coaching

Your practice is offering a new NHS service - Health and Wellbeing coaches. Health and wellbeing coaches offer a proven approach to help support patients with improving a variety of health issues and overall wellbeing.

Most of us know we need to adopt more healthy behaviours but find it challenging to put things into practice and stick with this. We may face stress, fall into bad habits or face long-term conditions we might find difficult to manage A Health and Wellbeing Coach takes the time to listen and work with you on the things that matter to you - not just medically, but in your life holistically.

At each session, you will talk about how your health and wellbeing is presently, set goals for yourself and be guided towards activities, tools, people and resources that can support you to make changes. You may be surprised that the following conditions can be helped by improved lifestyle measures and coaching: Psoriasis, Asthma, Fibromyalgia, Anxiety and depression, Obesity, Hypertension, Type 2 Diabetes, Osteoarthritis and some sleep conditions.

How does the coaching program work?

Coaching is based around what matters to you. A coach is there to support you in identifying what you would like to work on and help you reach your goals. Coaching is a collaborative activity, a coach will support you to achieve goals that are workable around your current situation and needs.

Your coach will find out more about you, your lifestyle, values, goals and anything that might get in the way of achieving those goals. You will then set some short-term goals that will enable you to start to work towards what you want to achieve. If the service is deemed a good fit for you, you will be offered 6 one-to-one sessions with a coach. These sessions last up to 50 minutes and can be done in person at a local GP surgery, by video call, or by telephone. The sessions are usually weekly.

  • What is coaching? Coaching is future-focused, and goal orientated and aims to support you to make significant, life-improving choices and decisions, whilst making you aware of limiting beliefs and challenging them so that you’re not unintentionally holding yourself back.
  • What happens after I finish the support? Your coaching sessions are designed to put you in a more confident place to self-manage your lifestyle changes. We know that sometimes we need a further helping hand. We can signpost you to community services that can provide long-term support where appropriate. Further support will be discussed during your sessions.
  • How much does it cost? This is an NHS service, so there is no cost. 
  • How do I join the program? You need to be referred by a member of staff at your GP practice (a Doctor, Nurse, Social Prescriber etc.).
